Personalized medicine, for which needs have been increasing in recent years, refers to selecting drugs and treatments tailored to specific patient populations and individuals and providing them more safely to the most appropriate patients. Prof. Yoshiyuki and Dr. Hiroshi Tamura wrote an editorial of the Special Issue entitled “Diagnostic Challenge and Therapeutic Approaches in Human Sepsis Based on the Appearance of Endotoxemia and Beta-D-Glucanemia” of Int J Mol Sci (Impact Factor: 5.924). There are 11 exciting papers, including five on endotoxins and six on beta-D-glucan. We hope that this Special Issue will be helpful for future research in Gram-negative sepsis and invasive fungal diseases. A scientific paper, “Low EndotoxinRecovery—Masking of Naturally Occurring Endotoxin “ was recently published by MDPIAG (Reich J, Alexander F, Tamura H, Nagaoka I, Motschmann H Int. J. Mol. Sci.2019, 20, 838). The Low Endotoxin Recovery (LER) phenomenon was first reported by Genentech Inc. in April 2013. Since then the Food and Drug Administration (FDA) raised concerns about the potential false negative LAL test outcomes due to the masking of endotoxins in various drug product formulations with time. Dr. Patricia Hughes of FDA/Center for Drug Evaluation and Research (CDER) gave a presentation on FDA regulations and endotoxin detection, taking into account the LER issue, at the Parenteral Drug Association (PDA) Europe Conference held in Berlin, Germany on February 17, 2015.

A meta-analysis of four decades of studies on the concordance between Gram-negative (GN) bacteremia and endotoxemia was published in the Journal of Clinical Microbiology (JCM 2015; 53(4):1183-91). This important article discussed the reasons behind the weak concordance of endotoxemia with GN bacteremia. It is considered that the LAL reactivity to (1→3)-β-D-glucans and plasma pretreatment techniques should have had a huge impact on the matter along with LAL sensitivity to endotoxins involving the choice of the LAL assay format.

Since September 2012, the Centers for Disease Control and Prevention (CDC) has been investigating an unprecedented outbreak of fungal meningitis associated with contaminated methylprednisolone acetate (MPA) injections used in epidural steroid treatments. By October 2013, 751 cases were identified and 64 deaths were reported in this regard, thus, making this fungal infection as one of the largest recorded healthcare-associated outbreaks in the history of the United States. It should be noted that a black mold, Exserohilum rostratum, was identified as the predominant organism involved in these cases. In addition, CSF (1?3)-β-D-glucan levels with FDA-approved Fungitell assay may contribute to the rapid diagnosis of fungal meningitis caused by contaminated MPA injections with considerably high sensitivity and specificity (Malani et al. 2015. J.Clin.Microbiol. 53:799-803; Litvintseva et al. 2014. Clin.Infct.Dis. 58:622-30).
